Client Service Agreement This Client Service Agreement is entered into by and between Uni Wellness (the "Company") and (the "Client Name") * Dated * MM DD YYYY The address of the location for the event or booking. * Date of Service and Service Requested * MM DD YYYY Deposit/Changes/ Cancellations and Refunds: All deposits are paid through a third party processor, Invoice ASAP. All deposits are to be paid prior to any confirmed bookings. To make any changes to your booking, service, or finalized menu: Uni Wellness requires a (7) day advance written notice of any and all changes. Last-minute changes without a(7) day written notice will result in an additional charge or service fee. If a client decides to cancel an event or booking within (30) days of the event or booking date, Uni Wellness will fully refund the Deposit. Cancellations made within (14) days of the event or booking date are 50% refundable. Any cancellations made after the (14) days of the event or booking are NON-REFUNDABLE. All cancellations require a written notice of cancellation sent and confirmed receipt to SUPPORT@UNIWELLNESS.LIFE. A signed client service agreement must be signed with all confirmed deposits. Food Safety and Sanitation: Uni Wellness chefs and staff are trained in a Servsafe Certification. Uni Wellness will inspect ingredients and supplies, we are not responsible for any unknown or unforeseen conditions. Uni Wellness is not responsible for any illness or death due to manufacturing flaws or recalled products. Final Payment/ Gratuity: Final Payment is required upon completion of an event or booking. The client will receive a final copy of the invoice, with all receipts attached. Final Payment forms accepted are Venmo, Zelle, Cash, or 3% fee for all credit or debit cards. There will be a $ 50-a-day fee added to each day the final invoice or balance is left unpaid. Gratuity is greatly appreciated by the Uni Wellness Team. For parties or events with 12 or more people, a 15% gratuity will be added to the total service fee. Client Repsonsabilitles: Please reference your finalized service menu or agreement, and or booking confirmation email for the listed responsibilities of ("Client'). Failure to meet the Client's responsibilities, can and may affect the result of the customer experience and performance or quality of booked services. Photography/ Video Disclaimers: Uni Wellness reserves the right to use any photography or video taken at any event without the expressed written permission of those included in the photography or video. Uni Wellness may use the photographs or video in publications or other media material used or produced or contracted by Uni Wellness, including brochures, invitations, books, social media, etc. Any person desiring to not have his/her photo taken and used as above described must notify Uni Wellness at Support@uniwellness.life, in writing prior to the event or booking and include an in-person photograph. Uni Wellness will use the photo for identification purposes and will hold it in confidence. Both parties here are to have executed and agreed upon this Client Service Agreement from the date first set above.
